If you use the contact form on this Site, we collect the information you enter: your name, phone number, email address, the reason for your inquiry, and any message you choose to write. Submitting this form is voluntary.
Information collected automatically
Like most websites, our hosting provider automatically logs basic technical information from every visitor as part of standard web server operation — things like IP address, browser type, device type, and the pages requested. We use this only for security and to keep the Site running, not to build individual profiles of visitors.
Analytics and Advertising Tracking

- Web3Forms — when you submit the contact form, the information you enter is sent through Web3Forms, a third-party form-delivery service, which forwards it to our office email. Web3Forms processes this data on our behalf and does not use it for its own marketing.
- Google Maps — an embedded map on this Site is provided by Google and is subject to Google's own privacy policy.
- Google Fonts — this Site loads typefaces from Google's font service, which may log basic request data (like IP address) as part of delivering the font files.
- Google Translate, Google Reviews, and social media links — these are outbound links to external Google and social media pages. Once you leave our Site, your activity there is governed by that service's own privacy policy, not ours.
How We Use Information
We use the information submitted through the contact form solely to respond to your inquiry, schedule or discuss appointments, and communicate with referring dentists. We do not use it for advertising, and we do not sell it.
